Article #139: fl_connect 0.99

A pretty complete rewrite of the callback system.  Got rid of the internal table, storing connection info in the widget instead.  This system is still completely compatible with the original FLTK code (the old callback system continues to work without alteration;  all existing code should compile without change). 

The API is unchanged from version 0.92, with the exception that we now support 0-parameter callbacks.

I like this version much better.  It's much more space-efficient.

Tested under gcc 2.95.3, 2.96 (urgh!), and 3.2.2.  Please, tell me that it compiles under Borland and VC++... ??
