Had some time -- here's a proof of concept using the above technique. I included your eye icon and made an eye open/closed version of it, and instanced it twice in the custom item, so each can be clicked to toggle them separately. tree-with-justified-eye-icons.cxx.txt

The screenshot shows where I clicked the right hand eye icon in MyItem-002 to toggle it.
I turned off the drawing of the selection color because I didn't bother to make proper alpha channels for the antialiased icons so they'd draw properly when selected, but you can do that with pngs.
